Please be sure to send in your child's most recent Physical Exam from between 9/6/2021-9/6/2022, as well as their most recent Immunization record. Remember that Flu shots are required up to the age of 5 years, unless they are exempt due to Medical or Religous reasons.

ATTENDANCE:If you are calling your child out from school due to illness or injury, call the Attendance department at extension # 1017
ILLNESS:If a student has a temperature > 100.0 F , the child may return to school 24 hours after the temperature returns to normal (98.6) without the use of medication (Tylenol/ Advil/ Motrin). These orders are written as per our school policy. Checking your child's forehead is not a good measurement of temperature, use a thermometer for accuracy. If a child has vomiting or diarrhea, he/she should be excluded from school until 24 hours after the last episode of vomiting or diarrhea.
MEDICAL INFORMATION:Any changes in your child's medical status need to be documented by your child's physician and given in writing to the school nurse. If your child has been diagnosed with a new medical diagnosis, if a physical is performed, or if any immunizations are given, please send in any health information to the school nurse. Please update this health information yearly.
MEDICATION:If you wish to have medications in the nurse's office, please call me, so I can send home the appropriate paperwork. Medications cannot be carried into the school by the student.
CHAPSTICK/ COUGH DROPS:Chapstick and cough drops cannot be brought to school. If your child has a persistent sore throat, call your Pediatrician because it maybe something more serious that requires medical attention. Chapstick can be applied before school. We have orders from our school physician to apply Vaseline to dry lips if needed. Please keep chapstick at home.
GYM/ RECESS EXCUSES:Parents/ guardians may write an excuse from physical activities for up to 2 days, after 2 days, a medical note is needed. If a parent/ guardianor physician writes an excuse from physical activities for gym class, please write a note if you want your child to sit outside or inside at recess. You may write it on the note itself. Students may sit outside and enjoy the weather with their friends at recess and not participate in gym class. A quick note by the parent/ guardian can eliminate phone calls.
INJURIES/ PLAYGROUND INJURIES:If your child has had an injury in school, or on the playground during school hours and it was not reported by the student, the parent/guardian should immediately notify the school nurse as soon as possible.Call the nurse's office and leave a message at extension #1073 or the front office at #1270
FIELD TRIPS:Field trips will be continuing throughout the year. If your child has an inhaler, EPI PEN or Benadryl, the parent will be asked to attend the field trip. If you are unable to attend, please contact the school nurse as soon as possible.
STREP:If a strep test is performed on your child and comes back positive, please call the school nurse.
LICE:If your child is positive for head lice, or if you are unsure about your child having head lice, please contact the school nurse, so I can follow up with your child. If a student is found positive for head lice, it is necessary for me to screen the whole class. I can also assist you with further information if needed.
SEASONAL ALLERGIES:Allergy season is in full swing. If your child takes allergy medication, please administer it as directed by your physician. It is very difficult for children to stay focused and benefit from their learning opportunities without proper maintenance.
EAD INJURIES:Head injuries can occur at any time during play or sports. A concussion is a type of brain injury that changes the way the brain normally works. You can't see a concussion. Signs and symptoms of a concussion can show up after an injury or may not appear until hours or days after an injury. Signs and symptoms include: headache, nausea/vomiting, double vision, changes in vision, sensitivity to light/sound, feels sluggish or foggy, difficulty with concentration and short term memory loss, sleep disturbance and/or irritability. If you suspect that your child has had a concussion or has these symptoms, seek medical attention immediately. http://www.cdc.gov/

WELLNESS SERIES:In the lobby of the school are pamphlets related to children's health and wellness. Please use these for your own knowledge.
